Job Description

Compensation:
We offer our employees a competitive compensation and benefits package that reflects our organizational culture, mission, and core values. The hourly range for this position is $21.40 to $25.15 and is commensurate with experience.

The Lead Crisis Patient Services Coordinator plays a critical role in supporting Federally Qualified Health Center (FQHC) operations by assisting clients in crisis and coordinating essential client services. This includes managing resources, facilitating insurance enrollment, and scheduling appointments. The Coordinator oversees the volunteer and intern team, triages immediate client case needs, and develops standard practices and resource guides to ensure a consistent and high-quality client experience across FQHC services.

Required: HS/GED; 2+ yrs. experience in a Primary Care office; 3+ years working with homeless or underserved population in DC; experience with eClinical Works EHR

Required License/Certification: CPR/First-Aid; certified in Mental Health First Aid and Handle with Care De-escalation (w/in 30 days of hire)

Schedule: Monday - Friday, 8:00 am - 4:30 pm (40 hours per week)

Expected Contributions: Crisis Support

  • Greet clients and visitors, identify their need and provide information and initial direction.
  • Provide positive initial customer service experience to all clients and visitors to SOME.
  • Contain, identify, and triage clients' most urgent needs.
  • Train and oversee volunteer and intern corps performing case work and client support.
  • Verify medical insurance and/or enroll clients in medical insurance.
  • Coordinate enrollment or participation in agency programs with clients and appropriate staff.
  • Offer all appropriate SOME services to each client seen

Administrative:

  • Answer phone line and direct calls
  • Scan registration and referral forms into EMR. Update EMR as needed with client information.
  • Perform weekly billing Audit
  • Provide administrative support to medical, dental and BHS as assigned
  • Ensure medical records and referral assignment are being completed by PSC.
  • Assist in providing direction and assistance to Front Desk team as directed by the Clinic Practice Director
  • Work directly with Director and Clinic Manager to develop and implement process improvement changes
  • After collaboration with provider, case manager, or supervisor - assist in coordination of patient emergency transportation needs to appropriate facility
  • Oversee insurance referrals and pre-authorizations

  • Knowledge of electronic health records management (specifically eClinical Works)
  • Knowledge of chart reading and common diagnoses
  • Good listening skills to identify client needs and direct accordingly
  • Customer service skills to work with clients (both patients and volunteers)

Reports to: Senior Director, Community Resources

Position Designation: This position is designated as Safety Sensitive. You may be subject to drug testing prior to or during your employment with SOME. In this position, you may be disqualified from employment based on the presence of marijuana in test results, even if you possess a medical card authorizing the use of medical marijuana.

Environmental Working Conditions: This position is in an office setting. This office may or may not include individual heating and air conditioning controls. This office may or may not include a window.

Physical Demands: Must be able to lift up to 20 pounds. Requires looking at a computer screen for several hours a day. May be required to sit for long periods. Must be able to travel to events and meetings offsite.
